What is BAUSCH + LOMB® horizon rewards?

BAUSCH + LOMB® horizon rewards is an evolutionary rewards program designed to build stronger relationships with our patients.

BAUSCH + LOMB® horizon rewards offers the opportunity to earn rewards for:
  • Purchasing our contact lenses & lens care products
  • Engaging with us through educational articles, videos and polls

Members can use their points and tokens for a great selection of gift cards (including the horizon rewards Visa® Reward Card), coupons for Bausch + Lomb® products and chances to win* exciting prizes!

What are the start and end dates for the Program?

The Program begins on July 1, 2017 at 9:00:00 AM CT and ends on December 31, 2024 at 11:59:59 PM CT. The Sponsor reserves the right to make any modifications to, or terminate the program at any time.

Who is eligible for BAUSCH + LOMB® horizon rewards?

This Program is open to all residents of the 50 United States, the District of Columbia, and Puerto Rico who are at least 18 years of age at the time of registration (19 in AL and NE), and have a valid email address.

How do I become a member of BAUSCH + LOMB® horizon rewards?

Become a member of BAUSCH + LOMB® horizon rewards by visiting the Program Website at www.BauschRewards.com and registering. Registration includes providing certain information about yourself. You will be requested to provide your full name, Zip Code, date of birth, email address and password, and confirm that you accept these Official Terms and Conditions.

Once you are registered, you can sign into your account at the Website with your email address and password.

How do you use my information? Do you share my information with anyone?

You can find our full privacy policy at https://www.bausch.com/privacy/.

We will use your information to administer this Program and as indicated in our Privacy Policy. We will not share your information with other organizations except as set forth in our Privacy Policy.

Are there any restrictions to this Program?

Participation in the Program is only available to registered BAUSCH + LOMB® horizon rewards members who are eligible to participate. Please note that you must be a legal resident of the 50 United States, the District of Columbia, or Puerto Rico, and 18 years of age or older (19 in AL and NE), to be eligible to join BAUSCH + LOMB® horizon rewards.

Points and Tokens earned in the Program are non-transferable and have no cash value.

There is a limit of up to 60 Tokens that Members may earn per year per Member Account/household for lens care product purchases. A claim submitted under one Member Account may not be submitted under any other Member Account.

Point-earning offers may have limitations that will be described on the offer materials. Contact your Eye Care Professional for more information on currently available offers.

Some limitations apply to all offers: for example, there is a limit of one (1) submission per person per 12-month period, a limit of five (5) submissions per address per 12-month period, and claims must be submitted within 60 days of the purchase.

Sweepstakes, Instant Win games and Coupons: No single person/Member Account may receive more than USD $75.00 annually from the total value of (i) Prizes awarded in a Sweepstakes and/or Instant Win Game, and (ii) Coupons.

Points & Tokens

What is the difference between Points and Tokens?

Points are earned by purchasing BAUSCH + LOMB® contact lenses. They can then be redeemed for a choice of Gift Cards, Reward Cards, Donations to Charity, or converted into Tokens.

Tokens are earned by engaging with BAUSCH + LOMB®. Tokens can be redeemed for BAUSCH + LOMB® coupons, or to enter exciting promotions* for chances to win prizes.

Where can I track my Points balance and Tokens balance?

Your Points balance and Tokens balance are always displayed by your user name in the top right corner of any page of the Website. You can also find these balances under the "Rewards" tab.

When will my Points/Tokens expire?

Points and/or Tokens in any Member Account where no activity (either earning activity or redemption activity) has occurred for twelve (12) consecutive calendar months will expire at the end of the last day of the twelfth month.

Earning Points

How do I earn Points?

Various Point-earning offers may be made available from time to time, whereby Points may be earned by eligible purchases of participating Bausch + Lomb contact lens products. Go to your eye care professional to obtain an Offer Code (and/or a Points Offer Form or other offer information materials, which will include an Offer Code) to be entered on the Website to start your claim.

Your claim must be submitted on the Website using the Contact Lens Points Offer Submission Form provided on the Website.

Your completed Claim Form, to be eligible, must be accompanied by:

(i)   a legible image of your eye care professional’s invoice (credit card receipts and packing slips will not be accepted), showing:
  1. proof of purchase of BAUSCH + LOMB® products (name and quantity)
  2. Purchase date within the time period indicated on the Points Offer Form or other offer information materials (purchase date will be determined by the date on the receipt submitted);
  
(ii)   a legible image of all eligible boxes and corresponding UPC(s) from all eligible product packaging.

What is the difference between a "New Wearer" and an "Existing Wearer"?

Some Point-earning offers may indicate that they are different for “New Wearers” and “Existing Wearers” of a participating product. A “New Wearer” is someone who has not previously purchased the respective INFUSE, Biotrue ONEday or Bausch + Lomb ULTRA lens (including modality, e.g. SVS, Astigmatism or Presbyopia) subject to the claim being submitted and who has not submitted a claim for rewards for such purchase to Bausch + Lomb Horizon Rewards within the two (2) year period preceding the current purchase of qualifying lenses subject to the current claim for rewards and an “Existing Wearer” is anyone who does not qualify as a New Wearer.

Where can I find UPCs to submit?

UPC Codes from participating Bausch + Lomb contact lens products must be submitted as part of a Points claim on the Website using our Contact Lens Points Offer Submission Form.

IMPORTANT: Before you attempt to submit a claim for Points, please visit your eye care professional to obtain an Offer Code (and/or a Points Offer Form or other offer information materials, which will give you access to the offer and describe the participating products as well as how to submit your Points claim).

UPC codes do not need to be submitted for Token-earning claims for lens care product purchases. Only your eligible receipt needs to be submitted through the Program Website.

My Contact Lens Purchase Claim was Approved, now what?

Congratulations! You are one step closer to redeeming for your choice of great rewards. Once approved, the points you were eligible for with your claim will be instantly credited to your account. Then it’s up to you to decide how to spend those points. Go to https://bauschrewards.com/spendactivities to find the reward that you want the most, or read our Spending Points section for more information.

My Contact Lens Purchase Claim was Rejected, now what?

If your purchase claim for Contact Lenses was rejected, our moderation team will provide you with one or more reasons why it was rejected. The most common errors we see are:

  • Not including an invoice that clearly shows the BAUSCH + LOMB® product purchased as well as the quantity
  • Providing a credit card receipt or packing slip instead of an invoice from your eye care professional

Within your rejection email you will receive a link back to your claim. This link will allow you edit any missing or incorrect information and resubmit for approval.

Earning Tokens

How do I earn Tokens?

Tokens may be earned by two (2) methods:

(i)   Purchases of Participating Lens Care Products: Tokens may be earned by eligible purchases of participating Bausch + Lomb lens care products.

Please note that any participating products available and corresponding Token values may be subject to change from time to time.

To submit a claim for Tokens, follow the instructions on the Website to upload a legible image of your receipt, showing your purchase of participating lens care product(s).

(ii)   Engagement Activities: New activities will be added to the Website from time to time, and may also be available on third-party websites or as special offers. Each Activity has a Token value associated with it.

Activities may be completed as indicated in the "Earn" section of the Website for the number of Tokens indicated; there may be limitations on how often a single Activity may be completed for additional Tokens. Activities may include, but are not limited to:

  1. Logging in to the Website (Tokens may be earned one (1) time per calendar month)
  2. Reading a short article (Tokens may be earned one (1) time per article)
  3. Opening an email (Tokens may be earned one (1) time per email)
  4. Watch a short video (Tokens may be earned one (1) time per video)
  5. Taking a quick survey (Tokens may be earned one (1) time per survey)
  6. Sharing with friends on social media or by email (Tokens may be earned three (3) times per calendar month, each time a friend registers for the Program using the Member’s unique link)
  7. Following the Sponsor on social media (Tokens may be earned one (1) time per social media platform)
  8. Entering an Engagement Code as distributed by Sponsor (Tokens may be earned one (1) time per Engagement Code)

Can I retake a survey after I finish it?

No, you can only respond once per survey. We will have many surveys available - check back often for new ones!

Where can I find promotional/engagement codes to earn Tokens?

Engagement Codes that can be entered to earn Tokens may be distributed in a number of ways including print advertisements, as well as communications made through the Website, email, and/or social media. Each Engagement Code can only be used once by a Member.

How many Tokens can I receive for lens care purchases?

Members may earn up to 60 Tokens per year per Member Account/household for lens care product purchases. Please note that a claim submitted under one Member Account may not be submitted under any other Member Account.

Spending Points

How to Redeem Points for a Gift Card or Reward Card:

(Tokens may not be redeemed for gift cards or reward cards.) Following the instructions on the Website, you may choose to redeem a certain number of Points (as indicated on the Website) for an available gift card. Items offered are subject to change at any given time, and are available while supplies last. Gift cards and Reward cards may be subject to certain terms and conditions set by the issuer. The availability of certain gift cards/items may vary throughout the term of the Program.

Check the "Rewards" section of the Website to see the currently available gift cards and reward cards and the required number of Points to be redeemed for each. You cannot redeem Points for an item unless you have enough Points for that item in your Member Account.

To redeem your Points for an item, browse the items available, select the item you wish to order and follow the instructions on the Website to confirm your selection. Once your order has been submitted, the required number of Points for that item will be deducted from your Member Account, and you will receive an email with instructions on how to claim your gift card.

Why would I want to Redeem Points for Tokens, and how do I go about that?

BAUSCH + LOMB® horizon rewards is all about choice. You are not just limited to one gift card

Some members may choose a blend of rewards with their points. As an example if I receive 600 points, I may choose to redeem for a $50 gift card, a coupon worth $3 off Biotrue® multi-purpose solution and a number of chances to win* a $75 Gift card. If I’m an instant winner - that’s $128 worth of value!

Some members will redeem their full amount right away, or you could build your points balance with multiple qualifying offers across the year and get something even more special. More tokens entered into our sweepstakes and instant win games give you more chances to win!

How to Redeem Points for Donations to a Charity:

(Tokens may not be redeemed for donations.) Points can be redeemed for donations to a charity which may be available from time to time in the “Rewards” section of the Website. The value of the donation to be made for each Point donated will be specified on the Website. Each time you make a donation, the applicable number of Points will be deducted from your Member Account.

Please note that you will not be sent a tax receipt for any donation made using your Points, but you can request one.

Spending Tokens

How to Redeem Tokens for a Coupon:

Following the instructions on the Website, you may choose to redeem a certain number of Tokens (as indicated on the Website) for an available Coupon. You may also choose to redeem Points for Tokens, which may then be used to redeem for Coupons. Each time you redeem for a Coupon, the applicable number of Tokens will be automatically deducted from your Member Account. You will receive an email with instructions on how to claim your coupon.

To Redeem Tokens for an Entry into an Available Sweepstakes or Instant Win Game:

Following the instructions on the Website, you may choose to redeem a certain number of Tokens (as indicated on the Website) for an entry into an available Sweepstakes* or a time-seeded Instant Win Game*.

Each Sweepstakes or Instant Win Game is subject to a set of Official Rules for that Sweepstakes or Instant Win Game. Official Rules will be available on the Website with details regarding eligibility, entry deadlines, prizes available, and how to enter.

Each time you enter a Sweepstakes or Instant Win Game, the applicable number of Tokens will be automatically deducted from your Member Account. There may be a limitation on the number of entries or game plays allowed per person for each Sweepstakes or Instant Win Game (as indicated on the Website). The number of Tokens required to be redeemed for an entry or game play into an available Sweepstakes or Instant Win Game may vary.

Promotions

How can I enter a Sweepstakes or an Instant Win Game without making a purchase or redeeming Tokens?

You may participate in a Sweepstakes and/or Instant Win Game available on the Website without making a purchase or Token redemption by following the instructions in the Official Rules for that promotion. Follow the instructions (in section #3 of the Official Rules for the promotion you would like to enter) in order to send in a request by mail for a Sweepstakes entry or Instant Win game play.

For Instant Win Games, please be sure to include the email address associated with your Member Account in your mail-in request so that we can send you back a link by email to play the game.

How can I see which Sweepstakes draws I have entered?

Click the My Account tab in the navigation bar. At the bottom of the page, under "Tokens Redeemed" you will see your current and past Sweepstakes.
MetaBank is not the sponsor of the sweepstakes. Funds expire on the date located on the front of the card.

How do you choose the winners of Sweepstakes promotions?

Winners are randomly selected in a draw from among all of the eligible entries received. Potential winners will be notified according to the Official Rules.

How are Instant Win Game prizes won?

Instant Win Game prizes are randomly time-seeded throughout the Promotion Period for the applicable Instant Win Game and are awarded to the first person who plays on or after the time each prize is randomly seeded.

Rewards & Prizing

When will I receive my Prize, Reward Card or Gift card?

Visa® Virtual Account

In approximately 48-72 hours of your win or redemption, your Visa® Reward Card order will be sent to our Visa® issuer, which will trigger an email from our Visa® issuer with a link and instructions on how to redeem the prize.

Visa® Virtual Account is issued by Pathward®, N.A., Member FDIC, pursuant to a license from Visa U.S.A. Inc. Terms and conditions apply. Virtual Account can be used online, via phone, or in store with a digital wallet everywhere Visa debit cards are accepted in the U.S. and District of Columbia. See Virtual Accountholder Agreement for details. No Cash or ATM Access.

Visa® Reward Cards

In approximately 48-72 hours of your win or redemption, your Visa® Reward Card order will be sent to our Visa issuer. Your Visa® Reward Card will be mailed to the address that was in your profile when your reward was confirmed. Visa® Reward Cards are usually delivered within 4 weeks to 6 weeks.

Visa® Reward Card is issued by Pathward®, N.A., Member FDIC, pursuant to a license from Visa U.S.A. Inc. Terms and conditions apply to Reward Cards. All other Gift Cards

In approximately 48-72 hours of your Prize win or Gift Card redemption, a link or an online claim code for the Prize or Gift Card will be sent to the email address you registered with along with instructions on how to redeem the prize.

How long do I have to use my Prize, Reward Card or Gift Card?

If there is any expiration date for your Prize, Reward Card or Gift Card, it will be indicated in the email you receive with your claim code and instructions.
